| Reporting forms self euthanasia|

Reliable information about self euthanasia (a dignified death or self deliverance in contact with close ones) is based on the most recent medical and toxicological knowledge and is updated with knowledge from practice. By gathering data on reports on self euthanasia by medication, by the voluntary refusal of food and fluid and by helium, information stays as recent and reliable as possible.

The reports on self euthanasia offer clear information on what medication has been taken, how long it takes to pass away, what symptoms might arise and how people present are being treated by police and justice. Failed attempts can also be reported, because this offers the opportunity to research what has lead to a failure of the attempt. And more important: with this knowledge possible failures of self euthanasia with others can be prevented.

Next to keeping reliable information on self euthanasia up-to-date, data is also being gathered for scientific research into self euthanasia. There is a wealth of research on suicides that are often characterised by mutilating methods in solitary and isolated conditions. There is also a wealth of research on euthanasia and physician-assisted suicide in the Netherlands. However, very little is known about the ‘road in between’, characterised by a humane way of ending ones life without violence and in contact with close ones. More research is needed to get an insight into the reasons for self euthanasia (e.g. the relationship with denied PAD requests) and to measure the impact on close ones that are left behind.

Stichting Waardig Levenseinde collects reporting forms on self-euthanasia.
The reporting form can be printed out and filled out, after which it can be sent to SWL, P.O. Box 3930, 1001 ASAmsterdam, The Netherlands. Filled out reporting forms can also be scanned and emailed to VasterlingCatharina@gmail.com
